What Is Hepatitis B?

Hepatitis B is a serious viral infection of the liver transmitted through contact with infected blood, body fluids, or sexual contact. It is 50–100 times more infectious than HIV. More than 2 billion people worldwide have been infected at some point, and over 240 million live with chronic hepatitis B — which can lead to cirrhosis, liver failure, and liver cancer.

Unlike Hepatitis A, Hepatitis B can become a lifelong chronic infection. HBV is the cause of up to 80% of liver cancers globally. There is no cure for chronic Hepatitis B — prevention through vaccination is the most important tool available.

Who Needs the Hepatitis B Vaccine?

Hepatitis B vaccination is universally recommended for all unvaccinated adults. For Ottawa travellers, it is specifically recommended for those:

  • Travelling to Asia (Thailand, India, Indonesia, Vietnam, Philippines, China, Cambodia)
  • Travelling to Africa (Kenya, Tanzania, Ghana, Ethiopia, Nigeria, South Africa, and across sub-Saharan Africa)
  • Travelling to the Middle East (including Egypt, Jordan, and Gulf states)
  • Travelling to Eastern Europe or Latin America for extended stays
  • Likely to have a new sexual partner during travel
  • Requiring medical or dental care in local facilities abroad
  • Engaging in tattooing, body piercing, or acupuncture while travelling
  • Healthcare workers travelling internationally
  • Anyone with chronic liver disease, HIV, or diabetes
  • Frequent short-trip travellers to high-risk regions

Even short-stay leisure travellers who might need emergency medical care abroad should be vaccinated — blood transfusions and surgical procedures in Hepatitis B-endemic countries carry real exposure risk.

Hepatitis B Vaccines Available in Ottawa

VaccineScheduleAges
Engerix-B (GSK)3 doses: 0, 1, 6 monthsAll ages
Recombivax HB (Merck)3 doses: 0, 1, 6 monthsAll ages
Heplisav-B (Dynavax)2 doses: 0, 1 month18 years and older
Twinrix (Hep A + B combined)3 doses or accelerated 4-dose18 years and older

Accelerated schedule: When travel is imminent, an accelerated 4-dose Twinrix schedule (days 0, 7, 21, and a booster at 12 months) can provide faster protection. Heplisav-B offers full protection in just 2 doses one month apart — practical for Ottawa travellers with less lead time.

Timing

The standard 3-dose series takes 6 months to complete. However, accelerated schedules provide protection in as little as 3–4 weeks. Ideally, start the series at least 2–3 months before travel to complete at least 2 doses and achieve good interim protection.

If you also need Hepatitis A vaccination, Twinrix covers both in a single series — saving you additional visits and reducing the number of injections.

Side Effects

Hepatitis B vaccine is very well tolerated. The most common reactions are mild soreness, redness, or swelling at the injection site, and occasional headache or fatigue. Fever occurs occasionally. Serious adverse events are extremely rare.

Frequently Asked Questions — Hepatitis B Vaccine in Ottawa

Is the Hepatitis B vaccine covered by OHIP in Ontario?
Hepatitis B vaccine is provided free of charge to Grade 7 students in Ontario through the school immunization program. For adults who missed vaccination, it is not covered by OHIP for travel purposes, though many extended health benefit plans cover travel vaccines. Ask your pharmacist about coverage options when you book.

I think I was vaccinated as a child — do I need it again?
If you completed a 3-dose Hepatitis B series after your first birthday, you are likely protected for life and do not need a booster. Check your immunization records — your physician can review these at your virtual consultation. If records are unavailable, a blood test (HBsAb titer) can confirm your immunity status.

How long does Hepatitis B vaccine protection last?
Following a complete vaccine series, immunity is considered protective for at least 30 years — and likely lifelong for most healthy individuals. Routine booster doses are not recommended for immunocompetent adults.

Can I get Hepatitis A and B protection in the same vaccine?
Yes — Twinrix (available at Orleans Travel Clinic Pharmacy) provides combined Hepatitis A and B protection in a single vaccine series. This is the most convenient option for most Ottawa travellers who need both.

Do I need the Hepatitis B vaccine for Thailand, Bali, or the Philippines?
Yes — Thailand, Indonesia (including Bali), and the Philippines are all intermediate-to-high Hepatitis B prevalence countries. Hepatitis B vaccination is recommended for Ottawa travellers heading to all three, particularly those with any possibility of medical care, tattooing, or sexual exposure during travel.

What if I need protection before my trip in 4 weeks?
An accelerated schedule is available. Heplisav-B provides full protection in 2 doses one month apart. Alternatively, an accelerated Twinrix schedule (4 doses over 21 days plus a 12-month booster) can be used. Discuss your timeline with your ISTM-certified physician at your virtual consultation — Orleans Travel Clinic Pharmacy has same-week availability.
